In exchange for allowing me to attend or participate in the programs operated by Manatee Adult Baseball, Inc., I, for myself and for everyone I may bind, release Manatee Adult Baseball, Inc., its representatives and the operators of the facilities upon which activities are conducted, from any and all liability whatsoever arising out of or in connection with my presence at, or participation in, the league's activities.

I understand that these activities involve physical exertion and risks of injury by actions, persons, equipment and other things. I represent that I am in good health and physically able to participate in all activities. I understand that the program may not provide any insurance except insurance to protect those providing the fields. I AGREE TO READ and to obey all rules.

I authorize the League to use my name and likeness in any League publications, including web site publications.

My check for the registration deposit ($60) payable to Manatee Adult Baseball, Inc., is enclosed. (Any difference between the deposit and actual registration fees will be applied to the game fees account.) All payments are non-refundable unless the Registration is not accepted. Once placed on a roster, I agree to pay for all games scheduled for my team, whether or not I am actually available to play.
